New Patients - Please sign in using the username and password you received in an email. If you did not receive, an email, please contact our Intake Department at 844-224-5264 extension 104 or email intake@communitybehavioralhealth.net

Self Screen - Please complete the                               before your evaluation by clicking the button and selecting the questionnaire that applies to the patient.

Existing Patients - Please sign in using the patient portal or directly contact your therapist or clinician from the staff directory.

There is a login issue for Apple phones or iPads, please use a computer to access your Patient Portal.
